2024 Rock County Community Health Assessment

The Health Equity Alliance of Rock County (HEAR) completed the 2024 Community Health Assessment (CHA) in July 2024. Healthy Rock will host the Rock County Community Health Assessment data and was developed as a quality improvement effort for the 2024 CHA. HEAR conducts a formal assessment every three years using the Mobilizing for Action through Planning and Partnerships (MAPP) framework.

The 2024 CHA was developed using the following tools:

  • Primary Data: Data collected by the Health Equity Alliance of Rock County through a survey, community conversations, and key informant interviews. Results listed below.

  • Secondary Data: Data gathered from credible external sources like state and national surveys.

    • Displayed throughout the Find Data pages of Healthy Rock

  • Historical Spotlights on Health: Showcases the impacts of historical events or policies impact health today.

    • Displayed throughout the Find Data pages of Healthy Rock

Results

Community Health Survey

A 32-question survey on perceptions of health that was available in English and Spanish. 537 total responses were collected throughout the survey period. More results located in the Community Corner section of the Find Data pages.

Community Conversations

Themes of the 2024 Community Health Assessment

A variety of tools were used to analyze the data collected for the 2024 CHA. The following six themes and subtopics rose to the top throughout the combined data sets. Topic guides are linked within the drop down boxes below for each theme. The community voted for the top two themes to become the priority areas of the 2025-2027 Community Health Improvement Plan (CHIP).

Priority Areas

The CHIP is a three-year action plan to better the health of Rock County. The first step is identifying the priority areas that the community identified as having a significant impact on Rock County residents. For this cycle, housing and mental health were selected. Collaboration and partnerships are essential to creating a successful set of goals and strategies to address the two priority areas. To view data on the two priority areas, use the “Find Data” page.
